Jenny Kolb, RN, BSN
Director of Ambulatory Care Management
Jenny, a registered nurse by trade, has more than 24 years of experience in population health and care management. With leadership experience in both hospital and ambulatory settings, she brings to FPAN a comprehensive understanding of the full continuum of patient care.
As Director of Ambulatory Care Management, Jenny enhances patient wellness and overall satisfaction by improving care transitions and reducing hospital readmissions. She primarily focuses on standardizing a system-wide approach to care management that streamlines processes for clinical teams while delivering a seamless, supportive experience for patients and their families. Her efforts demonstrate the value of care management and its role in driving meaningful improvements for the individuals we serve.
Jenny says her life’s greatest accomplishment is raising her three, wonderful children. When they’re not spending time with family, Jenny and her husband can be found enjoying all the outdoors has to offer— hiking, snowmobiling, boating, fishing, and reading under the warmth of the sun.
