Care coordination
We support clinical integration through a standardized approach to care coordination and population health strategies within our care management umbrella.
Patients and families often struggle with chronic or complex health conditions, developmental issues, or social service needs. Our team identifies community resources for patients and helps them navigate medical, psycho-social, financial, legal, mental health, and transportation issues.
Care coordination
Our care coordination team supports network plan members who need care coordination resources in our primary care designated medical groups. Utilizing multiple technologies and systems, the team provides telephonic, virtual, and in-person support to patients and providers. In addition, we provide nurse and social work case management on behalf of UCare for contracted members to support care transitions and long-term care coordination.
Medication therapy management
Our specially trained medication therapy management (MTM) pharmacists work closely with patients and providers to identify, resolve and prevent medication-related problems. Network providers and care coordinators can refer patients of independent Fairview Health Network primary care clinics for medication therapy management at no cost to the patient or the independent practice.
