FPAN honored at UCare’s “A Salute to Excellence!” awards ceremony

23 Minnesota clinics and care systems recognized for
outstanding results in 2023 HEDIS measures and innovation

Edina, MN, October, 2024 – Fairview Physician Associates Network (FPAN) was recognized for its outstanding quality at UCare’s16th annual “A Salute to Excellence!” event Thursday evening, September 19, in northeast Minneapolis. FPAN was one of 23 clinics and care systems with top results in 2023 Healthcare Effectiveness Data and Information Set (HEDIS) measures – including preventive care, disease management, behavioral health, maternity care, care transitions and medication management. FPAN was honored for improving the health of UCare members enrolled in Medicare plans. FPAN was recognized for their quality in stating therapy for patients with diabetes, plan all-cause readmissions, and the use of opioids from multiple providers.

FPAN President John Kvasnicka, MD, said, “FPAN is proud to receive the UCare Salute to Excellence award once again. This award is further evidence of the high-quality cost-effective care delivered by the practices in our network. We would like to thank all our practices for working with the network to meet our mission: Bringing stakeholders together to drive healthcare value.”

This year’s A Salute to Excellence! evening was attended by more than 70 honorees and UCare leaders. The evening opened with remarks from UCare President and CEO Hilary Marden-Resnik, followed by a lively conversation with Dr. David Hilden from Hennepin Healthcare and UCare Executive Vice President/Chief Strategy Officer Marie Zimmerman about health care advocacy and how providers can make a difference for their patients and their practice. UCare’s Innovation Award was then presented to Thrifty White Health by UCare’s Director of Quality and Population Health, Jamie Galbreath. Next, UCare Executive Vice President and Chief Medical Officer Tenbit Emiru, MD, PhD, MBA, honored guests with UCare Excellence in Health Care Awards for their quality results.
